Assessment/Evaluation Resources:  Note that evaluations can be costly, so it is important to find a provider who offers a sliding fee scale or will accept your insurance. 

Adults:

  • Iowa Vocational Rehabilitation Office (now under Iowa Workforce Development)
    • Phone:  (515) 281-4211
  • Colleges and Universities Disability Services or Student Accessibility Offices may have a list of providers who do evaluations; some colleges also have clinics or departments that offer evaluations.  Colleges also provide documentation guidelines for applying for an accommodation.  Here is a list of some of the colleges that provide such lists on their websites: (Note-you do NOT need to be a student to view these online lists)
    • Every college has a Disability Service or Student Accessibility Office.  If you are planning on enrolling, and you have or suspect that you have a disability, you should contact their office prior to the start of the school year.
  • Primary physicians may give information about local evaluation resources
  • Private psychologist or mental health clinic; University hospitals/clinics
  • The American Psychological Association has a free tool on their website for locating providers/psychologists.  You can search by zip code, concern or need, insurance accepted and more.  https://locator.apa.org
